'Attach' button in all Templates Not Operating

We recently upgraded to v3.1. now the 'attach' button does not operate.
When you select 'attach', another copy of the main Webmail or Webaccess
page opens. It is like the button/link points to the wrong page. How do
we trouble shoot.

Also, how do we get the system to see our logo. We have an image -
'logo.jpg' - in the logo folder.
