It could be one of several problems, including a damaged disk drive.

Which version of Netware are you using?

Is the server fully patched? Can you load the newer *.HAM drivers, such as
IDEATA.HAM in place if the old ISADSK.DSK driver?

Assuming nothing else has changed, it could possibly be a bad drive or
controller. Is it possible to move the drive to the second IDE channel?
This would require changing the driver load command to accommodate the
different port and IRQ.
