the advice of Bob and Andrew is surely true but you should also
a) check whether nwtape.cdm is really needed for HP-Dataprotector backup
software. If not please prevent loading this module.

Code Address: 8A558000h Length: 0000E6D0h
Data Address: 0009C000h Length: 000015A0h
b) please update the following modules (SCSIHD.CDM by applying SP8) and
QL2x00.HAM (version 7.00.04) from the QLOGIC-website and HPQCISS.HAM
from latest HP-Proliant Support Pack. Please do not load SCSIHD.CDM
and CPQSHD.CDM at the same time because it may lead to the ABENDs you
have suffered from. If you have QLOGIC-FC-HBAs and a non-HP-SAN you
are being connected to then most SAN-vendor recommend to load only
SCSIHD.CDM.
Please check that after applying an HP-Support Pack that CPQSHD.CDM
is not load by accident in the changed C:\nwserver\STARTUP.NCF.
