you didn't fix the newsuperior - it should be:
newsuperior: ou=LEERJAAR-3,ou=ONDERWIJS,o=HLW

This is incorrect - it should not include the CN, just its "parent"
container.

What I don't like about your LDIF here is the newsuperior. I'd expect it to look like this:

dn: cn=20090286,ou=LEERJAAR-2,ou=ONDERWIJS,o=HLW
changetype: moddn
newrdn: cn=20090286
deleteoldrdn: 1

newsuperior: ou=LEERJAAR-3,ou=ONDERWIJS,o=HLW
