If I interpret this correctly, what it means is that you should not
allow SMTP except from your mail server. That is easy to do. You
should have an outbound smtp filter exception (probably smtp-st) that
would simply need to be changed to call out the mail server's IP
address as a source.
If your mail server itself is compromised and sending spam, filtering
on BMgr can't help.
