custom filter exception - port 4000

I have Craig Johnson's guide for filter exceptions book, but I haven't
been able to figure this one out. I need an exception for this:

How should it be written?

it just a simple tcp dest port 4000 filter.

From private to public, tcp dest. Port 4000, stateful. I usually
specify source ports 1024-65535 for good measure.
