Looks like you have that one.
Device failure? Hmm I would low level verify from the controller level and
see if it still deactivates. If it does, then I would double-check the
cables, termination and hardware. Kind of looks like the drive is starting
or has gone south and needs replacement.

The latest for the 3.x series is:
31 Jul 1997 vrp386.exe 57K VRP386.EXE; VREPAIR.NLM for NetWare v3.1x
When you boot the server do you get the Adaptec controller screen that gives
the option of ctrl-a for host adapter access? If you do, then I'd run the
low level verify.
