Haraka does everything you need and more.

Yup, Haraka is your project. I've used it myself personally and was very happy with the results.

Hopefully the API now allows to .createServer() and have your code completely decoupled from the haraka source code?

It does and it doesn't. I haven't tried to make it work that way, so there's nothing explicit to do that. But it might work. I have no use for it doing that so if someone really needs it then they'll have to submit patches. Nobody has needed it badly enough yet by the looks of things.
