(3) Every 5 years, send MA these Docs to save Admin Fee by handle State Tax yourself

 If you have been in MA 5 years, and you have been saving the Admin fee (so you file your own State tax), you need do the Agreement again with MA (it is required every 5 years), just do the same as you did before (see below):

or fax the following documents to MA to save administration fee & NJ sales tax (for selling supplements)(as attached):

1.ST-3
2. Two documents you received from NJ Department of Taxation.
3. MA Sales Tax Agreement.

Basically, just click on obtain a certificate online, and just filled in Tax ID and business name, it will generate a page of certificate.
