Dear Sir,

                 Sub: Information under Right to Information Act, 2005-Supply of Ration card for Mr. Rayanna Naidu, St. Joseph Street, Pannur village, Tiruvallur Dt. -Information-Regarding-Reuested.

                 Ref: Application given to the TSO, Tiruvallur, by Mr.Rayanna Naidu, dated 8th Sep. 2011.
                         Ack. from the TSO, Tiruvallur,  No. 154/04 dated 8th September, 2011.


I request  to be supplied with the following information , as per the provisions of the Right to Information Act, 2005:
Mr. Rayanna Nadu, of Pannur , Tiruvallur Dt. has given an application to you on the 8th September, 2011 , which has been acknowledged by you in No. 154/04 dated dated 8th September, 2012.

1. What is the normal time limit for the supply of a Family card, if the application is in full shape and is in order?

2. Have you come across any defects in the application of Mr. Rayanna Naidu, and if so , whether you have returned the application to him for necessary corrections ?

3. What are the special reasons for delaying the supply of Family card to Mr. Rayanna Naidu for more than 15 months?

4. Who is the person responsible for the intentional delay in the supply of the Family card for more than 15 months. depriving the individual the basic privileges and benefits provided by the Government?

5. Has any action been taken against the Officers, or the Assistants concerned for the inordinate delay in the supply of the card?

6. Does it mean that no fresh Family Card has been issued for the applications received in your Office, after 8th September, 2012?

7. What is the possible date by which the Family Card will be supplied to Mr. Rayanna Naidu?

I request that the above information may please be supplied at the earliest , since the individual has been suffering for want of a family card for more than 15 months, for no fault of his own.I have affixed court fee stamps worth Rs. 10/ as per the requirements of the Act.
