REGARDING HEALTH CHECKUP

Dear members,

Health checkup is to be held on 11-12-2016 at Pentagon Academy, Coonoor. Time 8.00 am to 10 am. Members who are willing to undergo the health checkup should give their names on or before 8th Dec. 2016. You can include your family members and friends.

Payement Rs. 450/- per person

For members Confed will refund Rs. 200/-

I request members to pay the amount on or before 8th Dec. 2016 to our treasurer in Ooty, Coonoor members can pay the amount to the President or Secretary.

Members who are going to undergo health checkup, should finish their dinner before 8 pm on 10th Dec. 2016. Blood test will be done only after 12 hrs of fasting.

Members are requested to follow the instructions.

I have attached the file which contains, what tests are done.
