Dear All,
This is reminder on 1 year follow up that is due to be completed 1 year after the last date of the interview for each centre.
One year follow up is required to be completed for all patients to get their vital status and procedures or events following the date of the interview.
Please find attached the 1 Year Follow Up CRF and Data Entry spreadsheet with 2 examples entered. All the information from the CRF should be entered on attached spreadsheet. You should not enter this data on the RedCap database.
Please see below the table with the last date of the interview for each centre.
Centres that are already due to carry out 1 year follow up are highlighted in yellow,
Centres that will be due to carry out 1 year follow up soon are highlighted in green:
