Symptoms of Infertility

Infertility which is also known as sterility occurs when a man cannot get a woman pregnant after having unprotected sex with the woman. In women, it occurs when a woman cannot get pregnant after having unprotected sex with a fertile man. However, having unprotected sex once which does not result in pregnancy is not enough to establish infertility. There should be at least 6 months regular unprotected sex.

Not getting pregnant is not the major symptoms of infertility. There are certain symptoms that may indicate infertility both in men and women. However, these symptoms depend on the cause of the condition. There are many health conditions with varying symptoms that can cause infertility in any of the genders.

Symptoms of infertility in women

Ovulation disorders are the major causes of infertility in women. These disorders can strike up changes in ovulation and menstrual cycle of a woman. So, if you witness changes in your menstrual and ovulation period, it may be that you have a disease that can make it difficult or impossible for you to get pregnant. Some of the symptoms of such diseases include:

·         irregular periods

·         abnormal periods

·         painful periods

·         no periods

Hormonal disorder can also make a woman to be infertile. Hormonal problem may cause the following symptoms:

·         weight gain

·         acne and other skin changes

·         changes in libido

·         thinning hair or loss of hair

·         dark hair growth on the chest, lips and chin

Certain health conditions which can lead to infertility may cause symptoms such as

·         pain during sex

·         milky white discharge from nipples when one is not pregnant let alone breastfeeding

There are other things that can cause sterility in women and they have their own symptoms.

Symptoms of infertility in men

It is difficult to detect infertility in men. Some of the symptoms may not be noticed until when a man wants to start raising family. Just as in the case of women, sterility in men can be caused by a number of conditions each having its own symptoms. Generally some of the symptoms of infertility in men include the following:

·         changes in the growth of hair

·         small, firm testicles

·         changes in sexual desire

·         problem with ejaculation and erections

·         pain, swelling and lump in the testicles

When to see a doctor

As it has been said, one unprotected sex which does not lead to pregnancy does not mean that you are sterile. You can only seek for medical help when you have tried up to 6 months or a year depending on your age. Men who are not up to 35 years who have tried up to a year to get their wives pregnant should visit a doctor. Women who are up 35 years and above that have tried to get pregnant for six months should see a doctor.

Tests

There are various tests such as imaging, urine and blood tests are done in order to discover the actual cause of the problem. The health of the sperm will be determined through sperm analysis.

Infertility Treatment in Women

There are three categories of treatment of infertility, namely, medications to improve fertility, surgery and assisted conception. These three infertility treatment options are explained below.

Medications to improve fertility

Ovulation disorders are the major causes of infertility in women. There are some medications that can be administered to women that have ovulation problem in order to improve their ovulation which is the release of mature egg or ovum from the ovary to the fallopian tube to be fertilized by a sperm cell. Hormones called gonadotrophins partly controlled ovulation in women. These hormones are produced in the pituitary gland located under the brain.

There are some medications that can be administered to women to improve their fertility. These medications work on the pituitary gland helping it to produce more gonadotrophin. Below are some of these medications that can be used to improve the production of gonadotrophin.

·         Clomifene is taken to improve the production of gonadotrophin. The medicine block a feedback mechanism to the pituitary gland and this causes the gland to release more amounts of gonadotrophin hormones.  The excess amount of the hormone can cause the ovaries to ovulate.

·         Medications that contain gonadotrophins are administered to women with fertility problem. They are available in injection form and are utilized if clomifene does not work. These medications can also be administered to men with certain hormonal problems in order to improve their fertility rate.

·         Metformin which is a medication for the treatment of diabetes can also be given to women with polycystic ovary syndrome (PCOS).

If you want to improve your fertility rate by taking any of these medications, it is advisable that you meet and discuss it with your doctor.

Surgical treatments

Certain infertility problems are treated via surgery. Some of the problems that may be handled through surgery include:

·         Fallopian tube problems: some cases of infertility among women are caused by the blockage of the fallopian tube. Such problems can be corrected through surgical procedures.

·         Endometriosis is another cause of infertility that can be corrected through a surgical procedure. In endometriosis, the cells from the lining of the uterus do not develop at the right place. Instead, they develop in other part of the body. The condition can be corrected through surgical procedure.

·         Fibroid can also cause infertility. However, there is no generally accepted expert opinion on this. Fibroids are removed through surgery. If the fibroid is the cause of infertility, its removal will solve the fertility problem.

·         Polycystic ovary syndrome is another cause of infertility in women that can be corrected through surgery.

Assisted conception

As implicit from the name, assisted conception refers to any technique carried out in order to help infertile women to conceive. There are a number of techniques through which assisted conception can be achieved. Some of the assisted conception techniques include:

·         Intrauterine insemination (IUI)

·         In vitro fertilization (IVF)

·         Gamete intrafallopian transfer (GIFT)

·         Intracytoplasmic sperm injection (ICSI)

·         Embryo donation

·         Egg donation

There are other treatments for infertility in women. But it all depends on the cause of the problem.

    Infertility is a condition that no man who wants to father a baby will like to suffer from. Unfortunately, it has no visible symptoms except failure of conception. In most cases some men blame their wives for their inability to conceive until it is discovered through medical tests that the problem is from the man.

    There are certain conditions that may cause infertility in men. So, it is the cause of the problem that will determine the treatment options for the problem. However, in some cases, the cause of the problem cannot be identified. Your doctor should recommend treatments that may work whether the cause is identified or not. It is also recommendable to check the female partner. The woman should be given treatment if there is need for it. Treating the female partner will also help to improve the condition of the man. Below are some of the treatment options for infertility.

    Use of medication

    Infertility in men can be treated using certain medications depending on the cause of the problem. Infection of the reproductive tract can make it difficult for a man to get a woman pregnant. If your inability to get your wife pregnant is caused by reproductive tract infection, your doctor will prescribe antibiotic that will take care of the problem to you. However, antibiotic treatment may not cure the infertility in some cases.

    Treating sexual intercourse

    Infertility in men can be caused by sexual intercourse problems such as erectile dysfunction (which is also known as impotence resulting from the inability of a man to have penis erection) and quick ejaculation (ejaculating too soon before one wants).

    Surgery

    Certain causes of infertility in men are only taken care of through surgery. For example, the obstruction of the vas deferens can make a man to be unable to impregnate a woman. It can be corrected through surgery. Varicocele which is the dilatation of the veins associated with the spermatic cord in the testes is another cause of infertility in men that can be treated through surgery.

    Treatment of hormonal problem

    Hormonal problems can cause infertility in men. High or low levels of certain hormones associated with reproduction in men can lead to infertility. If your infertility is caused by such problems, your doctor may prescribe certain medications for you. The doctor can also recommend hormone replacement as a means of improving your condition.

    Assisted reproductive technology

    Assisted reproductive technology abbreviated as ART is a way of helping men who cannot get their wives pregnant to father children of their own. There are various ways through which it is achieved depending on the cause of the problem. The sperm is obtained from the infertile man or through a donor individual. It can be gotten through surgical extraction or normal ejaculation. When the sperm is obtained, it is inserted into the vagina of the women if the woman is capable of conceiving. It can also be used for intracytoplasmic sperm injection or in vitro fertilization.

    Testing and diagnosing Infertility in Men

    There are several tests that can be done in order to determine the cause of infertility in man. Some of these tests are costly and it is not all health insurance plans that cover such tests. If you are having fertility problem, you should meet with your doctor. Your doctor may recommend certain tests for you. At the end of these tests, the cause of the condition may be determined or not. Below are some of the tests that your doctor may recommend for you.

    General physical examination and medical history

    Here, your doctor will try to find out your medical records. He will examine your genitals. Your doctor will ask you various questions bothering on inherited conditions, sexual development, chronic health problems, injuries or surgeries that cause infertility problem and others.

    Semen analysis

    This has to do with the analysis of your sperm. To obtain the sperm for analysis, you may be required to masturbate and ejaculate into a special container which will be sent to laboratory for analysis. The lab scientist will examine your semen to determine your sperm count, your sperm motility and morphology. The semen will also be checked for infections. In most cases, sperm analysis may take time to be completed in order to ensure accuracy. If the end result shows that your sperm is normal, then your doctor may require your female counterpart to be checked.

    Scrotal ultrasound

    The doctor may recommend a scrotal ultrasound test to find out if there is any obstructions in the testicles that may be the cause of infertility. Other problems in the testicles can also be observed through this test.

    Transrectal ultrasound

    This test is carried out in order to determine if the ejaculatory ducts and seminal vesicles (tubes that carry semen) are blocked. The doctor will check your prostate by inserting lubricated wand into your rectum.

    Hormone testing

    Infertility can be caused by hormonal problems. Certain hormones like the hypothalamus produced in the pituitary gland play important role in human production. If there is imbalance in the level of these hormones, sexual development and reproductions may have problems.

    Post-ejaculation urinalysis

    Normally, sperm cells are pushed out from the penis during ejaculation. But in some men, the sperm will travel backward into the bladder causing the urine to contain sperm. Post-ejaculation urinalysis is carried out to determine the presence of sperm in your urine.

    Genetic tests

    Genetic causes can cause sperm concentration to be low. This can be discovered through genetic tests. The test will reveal if there are changes in your Y chromosome. Certain other congenital or inherited syndromes can also be discovered through genetic testing.

    Testicular biopsy

    This test is carried out in order to determine whether your sperm production is normal or not. If the sperm production is normal, the infertility may be caused by other problems like blockage of sperm tract.

    Other tests that are used to diagnose infertility in men are

    ·         Anti-sperm antibody tests

            Psychological Challenges of Infertility

            Infertility and its treatment cause untold pain and psychological trauma on the couples suffering from it. The result of a study of 488 women in the US who were undergoing stress reduction program shows that infertile women suffer the same level of depression and anxiety with those diagnosed with hypertension, cancer or people recuperating from heart attack.  Though no research has been carried on men to determine the level of psychological trauma being infertile cause on them, they still experience distress. However, their level of stress is not as intense as those of their partners especially if the problem is from their partner.

            Typical examples of reactions of people diagnosed with sterility include anger, shock, frustration, depression, grief, loss of self-confidence, self-esteem and sense of control over one’s destiny.

            Besides suffering emotional crisis, infertility can also rupture a relationship in many ways. It can lead to a breakup in marriage. Relationships with friends, family members and loved ones can also be ruptured by infertility. For example, experience has shown that some couples who are not able to have children of their own inadvertently avoid interactions with friends who have children or who are pregnant.

            Emotional or psychological trauma associated with infertility can be caused by a number of factors such as effects of drugs, financial worries, uncertainty of the result of the treatment being given and many others.

            Medication effect

            Psychological effects associated with infertility can be caused by certain medications and hormones administered to infertile couples. Clomid, Serophene or any other synthetic estrogen clomiphene citrate that are prescribed to infertile couples because they are capable of enhancing sperm production and ovulation can cause insomnia, irritability, anxiety and mood swings in females. Other side effects of some infertility drugs include mania, thinking problems, depression and irritability. These side effects are similar to emotional response to infertile couples. So, it is difficult for medical personnel and infertile couples themselves to determine which side effects are psychological and which one is as a result of the drug reaction.

            Choices and outcomes

            Infertility treatments have helped some to raise children of their own and yet a good number of infertile couples have remained without children after undergoing series of infertility treatment. Couples who finally become parents after undergoing treatments are always happy. But the condition of those who are not able to become pregnant becomes worst after undergoing various treatments.

            The uncertainty of the result of infertility treatment and treatment failure increases the psychological trauma in some patients especially those from Western world where it is believed that any hardworking person will always achieve their objective. Another problem caused by failed treatment is the difficulty in determining when to stop the treatment.

            Financial concern

            Infertility is not covered by most insurance companies in the US. Even in the 15 states that have obligated infertility treatment insurance, the extent of treatment varies. Besides, most infertility treatment options are highly expensive. It will leave most infertile couples financially drained. The financial concern associated with infertility can also cause its own psychological problems.

            A Look at the Boxing Day

            The world celebrates the Boxing Day on every 26 December which is a day that comes after the Christmas Day. It is a day of exchange of gifts. During this day, bosses present gifts to their employees, servants. Tradesmen also receive gifts as appreciation for good services. However, there is a deviation from the traditional understanding of the Boxing Day. It is seen by many people in the various countries where it is observed as a public or bank holiday that comes on December 26 or 1st or 2nd weekday following the Christmas Day. This depends entirely on the regional or national law.

            Each of the countries that observe the celebration may have its name for the day. In South Africa, the name has been changed in 1994 to the Day of Goodwill while in Ireland, it is called the Wren day or St. Stephen’s Day (St. Stephen was the first Christian martyr. The Roman Catholic Church celebrates the memorial of St. Stephen on 26 December). Most European countries including Netherlands, Scandinavia, Germany and Poland celebrate the Boxing Day as the Second Christmas Day.

            The Boxing Day is celebrated on 26 December in Canada. The Canadian government normally declares a public holiday on that day. All full-time workers in the province of Ontario Canada are given time off with pay.

            Brief history

            The actual origin of the term Boxing Day is not clear. Several theories have been put up to explain the origin of the festival but these theories are not definitive. The exact origin in the European culture is not known. But in the middle ages, the day was a day of alms and gift giving to the less privileged ones. The term is believed to be derived from the alms box used in places of worship for the collection of donations to the needy.

            However, the pre-Christian Roman Saturnalia was believed to have influenced the culture. During the saturnalia celebration, masters of slaves carried out the functions of their slaves. The celebration was also marked by benevolence and alms giving to the slaves. The Boxing Day celebration may also stem from the early Christian tradition of collecting gifts for the poor during the feast of St. Stephen which is celebrated on the 26 December in the Western church. On that day in the early Christian church, metal boxes were kept outside the church for the collection of special offerings for the needy.

            The Boxing Day is also linked to the older English tradition during which employees received boxes of gifts and bonuses from their employers. According to the English tradition, servants were also allowed to visit their families the next day after Christmas day.

            Today, the celebration has totally become a secular celebration with many nations declaring a public holiday to their workers. It comes up on the 26 December which is the day after Christmas and the day Christians celebrate the martyrdom of St. Stephen. In most countries, if the Boxing Day comes up on a Sunday, the public holiday will be observed the next Monday. In some countries like Britain and Canada, the day is known as the shopping day.
