Read code release format documentation

Hi folks,

I'm working on a Covid-related project that involves working with some oldish lists of Read v2 and Read v3 (CTV3) codes, and I'm looking for documentation of the data format.

The final release of v3 is still available on TRUD and I've managed to get a copy of the final release of v2, but the release downloads come with zero helpful documentation.  The tabular data files don't even have headers, and googling for filenames gives me nothing.

I've asked the Information Standards service desk but haven't had a response yet, so I was wondering: do any of the nerds on this list happen to have the historic documentation lying around?
