Payroll - vacation request form and vacation balances

I'm trying to create a form for employees to request vacation that
enables them to show their available vacation leave balance so that
the supervisor, when reviewing/approving the request, can make an
informed decision. I did create a new form, but cannot seem to allow
it to be used in the Employee Center role.
Alternatively, I cannot find a way to provide supervisors with
information on their staff's accrued vacation leave balances without
giving them access to the entire employee record (including tax
exemptions, deductions, etc.). I also have not been able to create a
report that provides this information to them.
I would appreciate any help.

Trolling through the usergroup I see some general comments on forms in the employee center
There's also one on vacation requests
This is probably an appropriate query to work with support on. Anyone else have some insight?
