Feedbackform in Popup when leaving the site

Hi all,

I would like to implement a Feedbackform onto my site.Since I am pretty sure
that I am not the first one with this idea, somebody might know where I
could find a already existing script.

It needs to cover the following functions:
- appears when I close the browser
- appears when I move on to an other site (domain)
- does not appear when I browse into the site (domain)
- possibility to shut down. (cookie...)

I would be very thankfull if somebody could help me find a script that
covers those functionalities or just part of it
