Form: sending to email not working

When I submit the form in the tutorial I get the following:
FormsEditField1=john ormsby
FormsEditField1=jk...@sympatico.ca
FormsCheckbox1=
FormsCheckbox1=
FormsCheckbox1=
FormsCheckbox1=
FormHandler1=Submit

Hi John
When you publish the form to email, these names should be acceptable.
You can rename the fields in the Properties palette to be more
descriptive if you choose. This output looks correct. Did you specify a
value for each of the checkboxes? Go to Page View, select one of the
checkboxes and see if there is anything in the Value field in the
Properties palette? This value is what will be shown below when someone
selects that checkbox. If none of the checkboxes are selected, the
values in the email will be blank.
