Adding components to device type appears broken in 2.11.8

Hi,

First time posting here, so please be gentle.

Has anyone else observed that trying to add a component (e.g. an interface) to a device type results in a web form that is empty except for the Create, Create and add more, and Cancel buttons? Started after upgrading from 2.10.x to 2.11.8 on two separate NetBox installations I maintain. Adding components to devices works fine; the issue only occurs in device types.

Yes they have.  It was the top pinned issue on github, and was fixed in 2.11.9.

However the issue is now unpinned so it's not so obvious at first glance.
