Job Title: Case Manager - Nutrition Program Assistant
Overview: Perform community coordination activities to improve the nutritional well-being for people living with HIV/AIDS in a comfortable, supportive environment. The ideal candidate will possess case management skills with a passion for helping people.
Minimum Qualifications:
1. AA or BA/ BS in health or human services field;
2. Two years’ experience working in the field of HIV/AIDS or with other chronic illness;
3. Two years’ experience providing community coordination services including referrals and successful follow-up for food, financial, medical, and supportive care services.
4. Experience with maintaining client records and writing progress notes summarizing client progress, goals, accomplishments, needs, and referrals
5. Good interpersonal, communication, writing, and computer skills.
6. Cultural and linguistic competence for the target population.
7. Clean driver’s license, ability, and means to travel on a flexible schedule as needed.
Responsibilities:
1. Tracks and documents client program participation;
2. Engages in community coordination activities, make and follow-up on referrals for community services, i.e. other food, nutrition, and financial resources;
3. Documents progress notes that summarize client progress, accomplishments, further needs, and referrals.
4. Bring a passion for helping our members in a compassionate, empowering, and supportive approach every day.
Status: Full Time – Full benefits including health, dental, flexible spending account (FSA), life insurance, retirement, and generous combined leave time. High thirties depending on experience.
