Avoiding pyramid elements in ICEM

Hello,

I am trying to create with only tetra (for the bulk) and prism (for BL) elements using ICEM. Is there a way to ensure there are no pyramids formed? I have a slightly complex geometry (image attached below) and I can't use extrusion to create the prisms. 

I have tried making the mesh fine without the element count going very high but it still creates around 400 pyramids for a 1 million element mesh.

I haven't yet tried it but would Trelis be better at creating such a mesh?

Create a single layer of prism, which can largely avoid pyramid elements. Then split the prism into multiple layers, which will work.

I fixed it and then converted the mesh by exo2nek, but got the problem of:
