Need Clarity on Patient Information Section content

OPE (Outpatient Encounter (NHCS-OPD, NAMCS, NHAMC)) 

For a NHCS report generated for a visit , need clarification on whether visit specific information or latest information from patient chart need to be captured.

• Patient Information section:
• Patient Residence Observation: Capture latest Residence type on patient chart  OR patient's Residence type at the time of concerned visit?
• Pregnancy Observation : Visit specific information, capture Pregnancy status/observation at the time of the concerned encounter. Please confirm
• Smoking Status - Meaningful Use (V2): Visit specific information, capture Current Smoking status observed at the time of the concerned encounter.Please confirm
• Tobacco Use (V2): Believe this is historical information. Capture latest Tobbaco Use information present on patient chart.

• Payment Sources Section:  Capture expected sources of payment for the concerned visit.
