Themajority of vehicle forms are created and maintained by the Iowa Department of Transportation (DOT). To ensure you have the most up to date information, the DOT forms linked below are downloaded directly from the Iowa Department of Transportation website.

Iowa DOT forms are in a fillable PDF format that do not open in most internet browsers so to make it easier for users to access these forms, the Iowa DOT includes the forms in a zip file that will download to the downloads folder or other drive on your machine.


Affidavit of Correction (Iowa DOT Form 411219)

Description: This form is used to correct information that has been written in error on a title, application for title, damage disclosure statement, or other document required for title transfer or when these documents contain an alteration or erasure.


Affidavit of Non Use

Description: This form is used if plates have not been surrendered for storage, registration fees have not been paid for more than three years, and the vehicle has not been operated or moved upon the highway during that period of time. If these conditions are met the owner of the vehicle may file a statement of non-use with the County Treasurer and the vehicle may be registered upon payment of the current year registration fee.


Application for Persons with Disabilities Parking Permit

Application for Replacement of Iowa Certificate of Title to a Motor Vehicle (Iowa DOT Form 411033)

Description: In order to apply for a replacement title in your county the title must have been issued in your county. If issued in another county you must contact that county. The county of issuance is shown on the title. This form is used to apply for a replacement title if your title has been lost or destroyed.


Certificate of Trust

Description: This form is a sample "Certification of Trust" document. A title application in the name of a trust and the transfer of ownership of a vehicle already titled to a trust are both required to be accompanied by a copy of the entire trust agreement. In lieu of requiring a copy of the entire trust, a "Certification of Trust" can be accepted as well. A "Certification of Trust" is a document that is simply a condensed version of a trust.


Heavy Highway Vehicle Use Tax Return (Internal Revenue Service Form 2290)

Description: Form 2290 from the Internal Revenue Service is used to figure and pay tax due on highway motor vehicles used during the period with taxable gross weight of 55,000 pounds or more. A copy of the submitted form is needed when renewing the registration on your heavy highway vehicle with your county treasurer.




Leased Vehicle Worksheet (Department of Revenue Form 35-050)

Description: The sale or lease price of vehicles subject to registration is exempt from sales tax. These vehicles are subject to a one-time registration fee equal to 5% of the sales or lease price. This one-time registration fee also applies when vehicles are sold between private individuals.


Leased Vehicle Worksheet for Move-Ins (Department of Revenue Form 35-051)

Description: The remaining amount of the lease due at the time a leased vehicle is brought into Iowa is subject to a on-time registration fee equal to 5% of the remaining lease price.


Lost in Mail Form

Description: This form is used by an applicant to certify that a vehicle document sent by the County Treasurer through the United States Postal Service was not received.


Non-Resident Military Personnel

Description: This form is used to certify that an individual registering a vehicle is a nonresident on active duty in the military stationed in Iowa.


Specially Equipped Vehicle Owner's Certification

Description: This form is used to certify that a vehicle has equipment installed to assist a disabled person or a vehicle is used by a person that has a wheelchair as their only means of mobility.

If you don't have the title, you can get an Odometer Disclosure form at your local vehicle licensing office. You cannot download this form. We will provide it for you in the office on tamper-proof paper.


*Note: Quick Titles aren't available for snowmobiles, vehicles or boats reported as stolen, insurance or wrecker-destroyed vehicles and boats, or vehicles with "WA Rebuilt" on the title.


It is highly recommended that the buyer and seller go together in person to transfer the vehicle title and registration (buyer) and submit notice of transfer (seller). This will ensure any discrepancies with the vehicle documents can be identified, and the seller can be confident that the ownership transfer is legally finalized.


All vehicles on Oahu are required to have a valid Registration, Certificate of Title, and State of Hawaii Safety Inspection Certificate. Description of each document can be under General Information below.


Over 1.2 million vehicles are registered in South Dakota, all of which are identified by a legal document called a vehicle title. As vehicles are bought and sold, the title becomes the legal documentation of each change in ownership. Both the buyer and the seller must follow specific procedures to ensure the title is correctly transferred from one owner to the next.


Effective July 1, 2015, vehicle owners have 45 days to apply for title and registration of an acquired vehicle. All vehicles acquired prior to July 1, 2015 must have been titled and registered within 30 days of the purchase date.


There is not a provision for extensions on the 45 day requirement. Interest and penalties will accrue on the motor vehicle excise tax. All fees are assessed from the purchase date regardless of when an applicant applies for title and registration.


Interest is at a rate of 1 percent or $5, whichever is greater, for the first month; and 1 percent per month thereafter on any application made after 45 days from the date of purchase. (Interest is not calculated on interest.) A one-time penalty equal to 10 percent of the tax or $10, whichever is greater, is automatically applied against any application made after 60 days from the purchase date.


South Dakota is a plate with owner state. This means that the license plates are retained by the owner. If you purchase a new vehicle, you will transfer the license plates from your old vehicle to your newly acquired vehicle. You can do this at your local county treasurer's office. Be sure to take your old plates with you to the county treasurer's office.


State law requires that upon the sale, transfer or trade in of a motor vehicle or large boat, the person who sells, transfers or trades the vehicle or large boat, must provide a completed Damage Disclosure Statement to the person to whom the vehicle or large boat is transferred. If a motor vehicle or large boat, which is titled in another jurisdiction, is to be titled in South Dakota, the person who registers the vehicle or large boat (importer) must also complete a Damage Disclosure Statement.


You will need your original out of state titles, a copy of your state driver license, social security number, and a Motor Vehicle and Boat Title & Registration Application. Take this paperwork to your local county treasurer's office to complete registration process. They will be able to tell you the appropriate fees pertaining to that county.


In South Dakota an ATV MUST be titled. If purchased in South Dakota, an ATV is subject to the 4% motor vehicle excise tax. However, if purchased by an out of state business, you will need to show proof of tax paid to your local Department of Revenue office.


ELT stands for Electronic Lien Title. When you have a lienholder on your vehicle loan, your title will remain in an electronic form only until the time your loan is paid off and the lienholder releases their interest in your vehicle. At that time a paper title will be printed and mailed to the location indicated by the lienholder in the release. South Dakota has been an ELT state since October 2012.


When a title record is issued with a lien or a new lien is perfected, the provider supplies the lender with an electronic message of the lien perfection. When a lien is needed to be released, the lender would send an electronic lien release to the Motor Vehicle Division through the provider. Providers also supply other services for their lenders.


The ELT vendors provide several different options for your business, so we would suggest that you contact them directly to determine the costs that will be associated with your participation in the ELT program.


Note: If a Georgia title has not been issued for the vehicle and the inheritor is not a legal resident of Georgia, the inheritor must obtain a title in their name first from their home state before selling or transferring the ownership of the vehicle to a Georgia resident.


Selling a vehicle can be simple. Before advertising the vehicle, make sure you understand state and federal requirements to avoid problems between you and the buyer. The following information should help.


Iowa has a staggered registration system. All vehicle license plates expire the last day of the month in which the vehicle owner was born. If there is more than one owner of the vehicle, they may choose which owner's birth month will be used. If the vehicle is owned by a corporation or company, the county treasurer will assign the month in which the registration expires. Once the birth month to be used is established, it will be used to renew the vehicle's license each year as long as ownership of the vehicle or any vehicle that replaces it does not change. Trucks and truck tractors over 5 tons gross weight are registered on a calendar basis.
