Anyone else unabel to add hosts through CSV upload?

Just finished a fresh install of Nagios and Nagrestconf on Ubuntu 16.04.

Everything seems to work, except the "Hosts" "Bulk Tools" > "Add Hosts" ability.

I choose a CSV with one line in it:

107310,10.0.0.1,hsttmpl-contacts,mgmt,local-checks

Log output is "Uploaded switchtest.csv"

Click "Apply Changes", and the Log output goes blank with the "Fail" message at the bottom.

Anyone else experience this, or have advice on where to troubleshoot?
