Tracking patient visits by all other LIPs other than Physicians

Hello all:
IS has been asked to create a report that identifies when all other LIPs (APRN, CRNA, PA, Physician Extenders) sees a patient within the hospital regardless of patient type. . Currrently our bylaws do not allow for these types of LIPs to sign orders they all must be co signed by a credentialed physician, therefore when entered into Meditech we enter them under the attending's name. They also do not have admitting privledges. We need to be able to capture this data on any patient that has a Hospital Account number and is seen by a LIP other than the physician. Also we are not live with PDOC therefore we have a hybird chart part electronic part paper. Physician documentation is still paper including orders. How are others capturing this data electronically. Any advise would be greatly appreciated.
