Composer v4 (14) TOC issue

Hi Fletcher,

Sorry to make this bug report here, but I kept getting errors when I tried to submit it through the support site.

When I installed Composer v4 (14) I noticed that if a heading contains a single or double quote, the TOC displays the HTML escape codes instead of a single or double quote. So There's becomes There#8217;s.

If any of my bug reports actually did get through on the website, then I should note that I figured out my other issue, regarding the first header in the document not being recognized. I decided to open that doc in TextMate and noticed there was a control character at the beginning of the line before the #, and that is what caused the problem.
