[MT-L] New Hospital - Meditech Setup ?

Hi all,

We are in the process of a building a new hospital which will replace our existing facility for inpatients, but the existing facility will still be used for outpatient and observation services. For those of you who have been through this, do you have any recommendations on how to set this up (eg... new database totally, new facility, new locations, etc?). We are Expanse, but recommendations from any platform would be applicable.

We actually haven't been through this, but our facility purchased a CAH 2 years ago. Unless there are specific reasons a new database is required, creating a new facility might be the least amount of work. Questions to ask that I would think of:
1) If a new database is created, how will you resolve patient medical record numbers between the two?
2) If a new database is created, consider any potential issues for ancillary department ordering and testing - will all procedures need to be duplicated for the new database? How would you assure they stay in sync?
3) If a new database is created, will procedure orders (i.e.Lab) from the new (database) facility need to be interfaced to the original facility?
4) Will the new facility have their own ancillary departments (i.e. will each facility have their own Lab, Radiology, Pharmacy)?

Even if a new database is created, the financial setup may be the easy piece to deal with. Look closer at the impact to ancillary departments and patient flow.

Hello,

We built a new hospital that opened in 2013 and are in the process of building another one that will open in 2026. In 2013 2 older hospitals were closed and for the 2026 build we are closing 2 hospitals, converting 1 to an ambulatory care only and opening 2 urgent care centers from rental space to replace the 2 closing ones.

While we are in the planning stage for the 2026 work we will be following the same model as we did for our 2013 one.

Note we are on the latest versions of Magic and ITS, not expanse (yet)

So, to answer your question, we treat new hospitals as entirely new meditech builds but we use a single Meditech database for our entire organization. They will have their own locations and facilities.

I can't recall all the reasons for doing this but some that come to mind are:
- We can build and test in Live without impacting/confusing the existing facility staff. The testing was very important as we had different workflows in the new site (mostly due to newer technology but also because of different layouts).
- having everything ready in live (without impacting staff) is important as there are more important things to worry about at live - like transporting patients from the old to the new hospital in a matter of hours - we closed down a highway and used a lot of ambulances!
- We needed to keep the old hospital information around for a number of reasons (financial, statistics). We removed front line staff access to the old facility/locations but left everything else as is. (this was very important around year-end)

We added a new hospital in 2013, so we just created a new facility which had its own locations etc. We used one MPI database, but used 2 ADM databases because they wanted the account numbers to be unique between the two facilities. In hindsight, that was a mistake so I do not recommend that unless you will NEVER need to transfer a patient from one facility to another for care during the same encounter.
