I'm not sure all this stuff about SMTP and default accounts is useful or
good advice. Most providers require you to use the SMTP they provide and
won't carry (they call it "relaying") messages from other domains. Start
removing SMTP servers and you'll kill your accounts.
Thunderbird's "default account" is useful if you /start/ writing a new
message while in Local Folders, where there is no associated email account.
If you /reply/ to a message, it should use the incoming "to" address as
the new outgoing "from" address.
If you start a message in a "real" account's folders it will use that
account for the outgoing "from" identity.
My best suggestion for a default account is to use the Folder Account
add-on, which lets you associate any folder with a specific identity,
and it will then use that identity as the outgoing address when starting
a new message, and optionally, when replying to old messages.

Apart from that there are many add-ons which will nag or warn you, or
help you with colour coding to assist in spotting when you're using an
inappropriate outgoing "from" identity.
e.g. Correct Identity, Confirm-Address etc
