My system has a firewall, all virus software is up to date and runs
often. It's not everyone in my address book, just what seems to be a
random other, every once in a while. Could there be something in
"nickname" that does this. I often send emails by "reply" function, but
almost never by "reply to all."
One other, unrelated question. Some emails appear with the "reply,
forward" etc. ikons above the review pane or above the header. Others do
not. Does that have something to do with how it was sent originally, or
is it something on my end?

If I understand you correctly, then the reply/forward icons are next to
the subject and mean you replied to or forwarded that message already.
One possibility is that when you reply to an email that was sent to you
and someone else, your reply goes to both the original sender and the
other person. Reply To All would include all CC adresses listed in the
original, regardless of whether or not they are in your address book.
Just Reply will send your reply to everyone listed as To, but not CC.

That is incorrect.
Pressing reply: Does as supposed to: reply to the SENDER
Pressing reply to all: Does exactyl that: replies to ALL: SENDER, TO's
and CC's
(And never will to BCC's, because not visible to a receiver.)
Just to clarify.
