Howto refresh favicon.ico?

Okay, this has been bugging me for a long time.

Occasionally when I'm working on one of my sites, I replace the
favicon.ico file that is displayed in the address bar for the site in
Firefox. The problem is that Firefox caches this icon in some
mysterious place, and nothing I can do seems to clear it out. I can
delete every iota of personal information, yet still, it shows me the
old icon in the address bar. As far as I can tell, it's permanent.

I need my address bar to show the icon that everyone will see (well,
except for the people who have already visited the site using Firefox,
I suppose) when they come to my site. How can I delete the cache of
address bar icons?

And while I'm at it, ditto bookmark icons. Once they get set to
something, it seems that there's no way in hell Firefox will let you
reset it if the site's icon changes short of deleting your bookmark
and recreating it. (Which, of course, won't help as long as it has
the old icon cached as described above...)

Clear "CACHE" under private data & hit refresh. Favicon should update
instantly!

If you load the favicon url in firefox, it'll refresh the cache, at
least for the address bar. I dunno about the bookmarks, though.

The favicons seen beside each bookmark, are stored in the actual
bookmarks file. There is an extension, called "Favicon Picker 2", that
enables you to change the icon, or remove it (causing it to refresh,
next time you visit that bookmark).
