where are URLs and address bar history stored ?

I have ran the delete everything from within setting, but there are
still loads of old, some over 2 years old, address/URLs in the address
bar when I type new URLs in.

I can't seem to find where to delete 'everything'

What version of Firefox are you using? Firefox 3.5 and later made it
easy to get rid of the addresses in the address bar dropdown. With
those versions, clearing the history will clear history from the address
bar list. Not sure about bookmarks, though, because I made sure that
none were ever listed there.
Those addresses are stored in places.sqlite, which is where the history
and the bookmarks are stored.

Yer the drop down list I'm on about not bookmarks.

Try this:

Go to Tools > Options > Privacy > History, and uncheck the
Remember...history boxes. Shut down FF, restart, all the histry should
be gone. Now check whichever remember... boxes seem useful to you.
