You did not mention where you got it from. I would ignore it.

If this was in a targeted email to your address, when you (or anyone in
the group) clicks on it, you identify yourself to the sender, with that
key number I've snipped. If anyone else clicked it, you have been noted by
the site.

A whois shows that it's been registered since 2005 but the owner is hiding
behind private domain registration. Most legitimate companies don't do
that.

Typing sgizmo.com in browser redirects to http://www.surveygizmo.com/ On
the face of it, it looks like it might be real. I'd still ignore it.

So the answer to your Subject line question: can't tell for sure.

It's a Survey Gizmo survey. According to
<https://www.mozilla.org/third-party.html>, Survey Gizmo's service is
used by mozilla.org and support.mozilla.org. That Mozilla page links
to <http://www.surveygizmo.com/> -- both that domain and sgizmo.com are
registered via the same anonymizing service.
