I am traveling at the moment, excuse the brevity of my answers:
(1) It doesn't really matter who (as in "which person") applies. You should have the person apply who represents the project best.
(2) You should ask for the amount of money you need – it has nothing to do with being more or less well known; make sure you have a solid case for your budget.
(3) There is little difference between individuals and/or academic institutions. You need to demonstrate that the project is solid and supported – that might be through a group of individuals as well as through an institution.
(4) It surely doesn't hurt to have well known folks in the FLOSS community support your project.
(5) If you have multiple endorsements, I would list them all.

Note that midnight tonight (30th November) is the application deadline
for the next quarterly round; failing to hit that deadline will result
in a significant delay in the consideration of your application.
