Thanks to all who read and commented. I updated the document[0] today with
suggestions; filed a BMO bug to have our feature form adjusted[1]; and
ported the process to the new MDN wiki[2].

The new form is live on the BMO dev server. If you are someone who submits
feature requests for MDN, please take it for a test drive. Mention broken
things in the bug. Thanks!

The other issue I hit was that I wanted the "Who has this problem?"
question to be checkboxes, not radio buttons.
