I managed to find the problem -

It turns out that either FreeType or possibly something using FreeType
(Mozilla?) has a problem with font hinting as supplied by fontconfig.

I turned off the hinting completely and discovered that all of the
kerning/size issues disappeared. It even improved kerning on my Debian
etch installation.
