Thunderbird 3/comm-1.9.1 now locked down for RC 1

We've now passed the code-freeze deadline for Thunderbird 3 RC 1.

Although there are still some blockers outstanding that we will be
fixing before we release RC 1, I am locking the tree down further to the
following rules:

- Patches on blockers can land without further approval.
- Patches with approvals can land, but only with explicit approval from
me (the release driver).

If you're requesting approval on a patch, it should land on trunk at the
same time or before requesting approval.

However, we're really trying to lock down for RC 1 so we're unlikely to
take any extra patches other than the ones on blocking bugs (e.g. they
will need to be very-near to blocking bugs for us to take them).
