Hello everyone,

I'd like to ask a question about ICD 10. Sorry, sorry, sorry!

To help with our transition to ICD11, we're improving our dictionary. And we have a problem with the EPSTEIN BARR virus. How is it coded in your system? B34.8 or B97.8?

If you have an EBV-induced lymphoma, do you code it as C83.7 only or C83.7 + B34.8 or C83.7+B97.8?

C83.3 for Epstein-Barr virus-positive diffuse large B-cell lymphoma (ICD-11: 2A81.6) https://www.orpha.net/en/disease/detail/289661

B27.0 for Chronic Epstein-Barr virus infection syndrome  https://www.orpha.net/en/disease/detail/2566?name=Epstein-Barr&mode=name

 

In coding our data, we use the following.

 

EPSTEIN BARR

B338

EPSTEIN BARR LYMPHOMA

C859¹

B978

 

¹ without further subtype information.

Good day
My suggestion is as follows:

1)        Infectious mononucleosis is caused by the Epstein-Barr virus (EBV), although a milder form is caused by the cytomegalovirus (CMV). Both EBV and CMV are members of the herpes virus family.

B97.8  (=Other viral agents as the cause of diseases classified to other chapters) is not a suitable code for EBV infection.

B34.8 (=Other viral infections of unspecified site) is  not a suitable code for EBV infection.

B27.0  (=Gammaherpesviral mononucleosis = Mononucleosis due to Epstein-Barr virus) is a specific code for EBV infection causing mononucleosis.

B27.9 (=Infectious mononucleosis, unspecified) is also a suitable code for EBV infection, although it is a residual code.

2)        Except for human immunodeficiency virus [HIV] disease, no infectious or parasitic disease will be accepted as causing a malignant neoplasm. So, EBV-induced lymphoma is coded as C83.7.
