Hello Team Leads,
I hope this finds you all doing well and getting excited about our upcoming flight week. We are processing our required paperwork differently this year than in prior years. In addition to requirements on the website for documentation we have the forms below that must be completed. Please review this information carefully and complete as soon as possible but no later than the deadline indentified below.
All Participants:
1.) Mishap Form: Must be completed by ALL FLIGHT WEEK PARTICIPANTS.
Flyers and Alternates ONLY:
2.) Disclosure Form: Document used to be completed during flight week, but we would like to try completion online and prior to flight week.
3.) Physiological Training Briefing and Signature Form: Previously held during flight week, but the class will no longer be offered. Review and completion of online briefing and signature of completion page now required.
Each team lead needs to collect each set of documents and return in one e-mail. For example, one e-mail for mishap forms, one e-mail for disclosure forms and one e-mail for physiological briefing. All documents are to be sent to both Kerry Mcmanis (copied on this e-mail) and myself by noon, CST on Wednesday, May 8th, 2013.
When saving each form you should save it by your last name and the file type. For instance, If I was saving these documents I would put: “Semple – Disclosure”, “Semple – Physio” and “Semple – Mishap” for the three different forms and then the team lead would send three separate e-mails with these documents.
Mishap Form:
Please see attached both the form and instructions for completing the form. Keep in mind the entire form most be done electronically and all blanks must be completed.
Disclosure Form:
Each flyer and alternate must read Appendix C of the Aircraft Operations Management Manual and sign and return the disclosure form. Please read the following article:

Physiological Briefing:

Aerospace Physiology, Spatial Awareness Briefing
When complete, each person must print off the last page, sign and return to show completion and understanding of the briefing. This information in this briefing will be reviewed with you just prior to your flight during your preflight briefing.
Once again please e-mail both Kerry and I the completed documents for Disclosure Forms and Physiological Briefings by noon, CST on Wednesday, May 8th.
Collecting this required documentation prior to the flight week will allow us more time during to focus on having an amazing experience while you are here in Houston and at Ellington Field.
