Thepurpose of the Texas Birth Registrar Certification is to improve birth registration in Texas by standardizing data collection and entry. Improving birth registration is critical to make sure Texans receive valid birth certificates, and that the data represents their health.

Birth records provide important public health information. The data must be accurate, complete, and measurable. Statistical data impacts the general health of Texas because it relates directly to research, public health programs, medical practices, and funding.


The Vital Events Data Management team develops, analyzes, and distributes public health data from records of vital events. The team also replies to statistical data requests and develops the Texas Vital Statistics Annual Report.


The birth registration team is responsible for registering live births filed by hospitals, midwives, or other individuals who deliver in Utah. A date of registration is assigned for each live birth certificate and any supporting documentation that is needed to register the birth certificate such as a paternity declaration. The registration team registers both current birth events and events that are older than one calendar year and have not yet been registered with the Office of Vital Records and Statistics (OVRS).


All live births filed with the Office of Vital Records and Statistic (OVRS) for individuals one calendar year or older shall be filed as delayed. Even if you have never obtained a certified copy of your birth certificate, or know that you do not have a birth certificate on file, you will need to start the process by completing an application for a certificate of live birth. Order a Birth Certificate online or print a paper application. If applying by paper application follow instructions provided on the application, and along with picture ID and the required fee, mail the form into OVRS. OVRS will contact the applicant for further information and steps to be taken if a delayed birth certificate needs to be filed.


Utah law allows for the registration of early-term stillbirth deliveries for pregnancies between 16 - 19 weeks gestation as calculated from date of last normal menses period to the date of delivery. The request form requires a Utah licensed physician to sign. Follow instructions provided on the application, and along with picture ID and the required fee, mail the form into the Office of Vital Records and Statistics. After you finish using the online 5-in-1 newborn bundle, mail the additional form and required evidence to the Office of the Registrar General to complete the registration. All parents must be present when using the online 5-in-1 newborn bundle in order to agree to the single name.


You can register the birth of a baby with a last name that is different from the parent(s) last name using the online 5-in-1 newborn bundle. All parents must be present at the time of registration to agree to it.


If the birth involves a surrogate, make sure that you have the following forms completed, signed and commissioned before starting the online registration. You will need to mail them to the Office of the Registrar General to complete the registration.


Completing this paperwork will ensure that when you come in to the hospital to deliver, the Admissions office will have all of your admission paperwork completed and ready for your admission. Please download and complete each form below and bring them with you to your Pre-Birth registration:


This document is the hospital notification of the newborn's birth. This is the form that will register your child in DEERS, TRICARE, Passport, Consular Report of Birth, and Command Sponsorship. Required Documents:


The Department of State has a new online application system for a Consular Report of Birth Abroad. the eCRBA does not require any new information or processes -- applicants enter the same information in eCRBA as in the paper application. This new application method, however, will allow you to enter your data, upload required documents, pay the fees and received expedited processing. Additionally you will be able to check the status of your application online.


**PLEASE NOTE** In order to use eCRBA and skip the interview, you will need to provide your PPT Acceptance Agent's email address in your online application when it asks for Contact Information for Parents.

Q. What documents are required for Pre-Birth registration appointment?

A: Military ID Card(s), BOTH parents' passports, birth certificate original or certified copy, and marriage certificate original or certified. AE 40-400B with section II completed and Full registration form completed.


Q. What does the U.S. Physical Presence requirement mean and to whom does it apply?

A. The US Department of State requires all parents to supply proof of living in the US for at least 5 years and with 2 of those 5 years being after the age of 14. This can be an ERB/ORB/SURF, High School Diploma, College Diploma, Tax Returns. It is part of the Consular Report of Birth Abroad (CRBA) application.


Q. If my child is born in Germany is U.S. citizenship automatic?

A. This answer is very subjective. As a general rule, citizenship is passed from Mother to infant. If the mother is not a US citizen, or cannot prove 5 years physical presence in the US, then the citizenship requirement will fall to the Father. If neither can prove the 5 year physical presence rule; citizenship comes into question. It is best to speak with a Passport Agent or an Immigrations Officer at the Frankfurt US Consulate.

The Certificate of Hawaiian Birth program was established in 1911, during the territorial era, to register a person born in Hawaii who was one year old or older and whose birth had not been previously registered in Hawaii. The Certificate of Hawaiian Birth Program was terminated in 1972, during the statehood era.


Any person to whom a Certificate of Hawaiian Birth has been issued may submit a request to amend an entry, including a legal change of name, on an existing Certificate. A request to amend a Certificate of Hawaiian Birth will, however, be considered to be and treated as an application with the Department of Health for registration of a late certificate of birth in current use, unless a standard birth certificate for that person already exists in the vital records of the Department of Health.


If the amendment request is subsequently withdrawn, all documents received in support of the amendment will be returned. If the requestor elects to proceed with the application for registration of a late certificate of birth, the documentary evidence submitted in support of registration will be reviewed and evaluated for adequacy. If the application is approved, a late birth certificate will be issued and the original Certificate of Hawaiian Birth issued to the applicant must be surrendered to, for cancellation by, the Department of Health. No filing fee is charged for the late birth certificate.


Should there be a situation of dual registration, the requested amendment will be made to the standard birth certificate on file if the required documentary evidence in support of the amendment has been submitted and evaluated to be adequate. If there is no standard birth certificate on file, an applicant is required to submit documentary evidence of the birth facts necessary to support the registration of the late certificate of birth. If approved, the late birth certificate will be registered in place of the Certificate of Hawaiian Birth, which must then be surrendered to the Department of Health.


The Vital Records office only registers births for the City of San Antonio and Bexar County. To get a birth certificate for a baby born without a medical professional, you and your baby must visit the Vital records office.


No: We will need an Original Notarized Affidavit with ID of person (other than the parents). The notarized letter must include the mother's name, with the address where the child was born.


If you've recently given birth in Victoria, you'll need to register your newborn baby with the Registry of Births, Deaths and Marriages. All births must be registered within 60 days of the child being born. There is no fee to register.


When you register the birth, you don't automatically get a birth certificate. You can order a certificate at the time of registration or order one separately later. We suggest you discuss this with the other parent.


For commemorative certificates, you may only choose standard post. This is because they're not official identity documents. Legal and commemorative certificates will arrive separately.
