Data not imported with Wells shapefile Modflow 6 Model Muse 4

I imported the wells shapefile but the data I want (depth and water level) are not showing up. Heres what I did after importing shapefile window

1. In Option 1 (checked set values to intersected).

Question- For Number Z formula --Do I leave it as Zero or One?
                   Does 0 mean Model_Top or Depth (from imported data)?

I want to choose One. and make...... = Model_Top (is elevation) minus Depth 

(to give me location of the well)

2. In Data window

I checked Depth and Water level to import both. 

Im confused about the Data Set drop down list, should I leave it as "NEW DATA SET" or choose one of the layers listed.  How does this affect the Z = 1, if done in part 1?
