Ho to fix that issue? Underextrusion when the extruding begin

Hello to all,
it's been a while that I don't post anymore here.
In the meanwhile I've been using my MOD-t as usual and moved on Cura 4.0 to slice (I'll post in the following days the accurate guide to my usual page https://www.thingiverse.com/thing:1535333 .
Since the moving from the old Cura 15 I ever had a very annoying issue: when the printer begin to extrude the filament is nearly not coming out to increment in the following millimeters and then become normal. This EVERY time the printer starts to extrude after a even short pause.
The result is that small islands like supports for mounting electronic (like raspberry cases, for examples) or small holes are very bad and not resistant as they result much underextruded. Often the first line of a inner small perimeter is completely not extruded.
I remember that Derek told me that he was working on a new path planner to resolve this issue but that never came out, even with the last firmware that came with the final version of the tool that didn't changed.
I tried to dial some parameters like jerk, acceleretion, overextrusion after z hop and so on without any luck.
Is there anyone between you that could fix that issue? If so please give me a hint!!!
Maybe Derek is still reading this group, even if you have a new work you can be of valuable help!
