Can you try loading your site with
and look at the resulting HTML with view->Page Source

For combine_css, at least, that should tell you, in HTML comments, why some of the external files are not being combined.

    ModPagespeedEnableFilters combine_css,combine_javascript

In the documen source I can see the pages have been rewritten but have not being combined (URLs have been rewritten, but they are still listed separately), so I decided to tesst also the latest beta version (1.5.27.2-2912), but still it not gives me the combined requests for CSS and JS.

Does someone has the same problem?
