I've noticed that it only works when there are cards with the same foreign language word (using the vocabulary type), but not when there is a duplicate english translation, even when the activated cards are all english-foreign language. It would be really handy to have this too

It's a feature, not a bug :-)
If you have two card types for different languages, you don't want a
card 'house - maison' to be marked as a duplicate of 'house - casa'.
So, it's best to put all words from the same language in the same card
type.
As for synonyms, i.e. the same foreign word having two different
translations, that's best handled manually as opposed to automatic
detection, since it's very rare that words are exactly equivalent, but
depend on the context.

What if it's just checking for duplicates within, say, English - French (so each language has its own card type)? For example, two cards being brown-brun and brown-marron don't show up as being duplicates, yet they're in the same card type.

The check is on the 'foreign word' field, and brun and marron are there
different words. And although you could both translate them as brown,
they are in fact not interchangeable :-)

I suppose so, but then it'd still be useful for the cards to be marked as duplicate so that I could change the meaning of one (or both) of the cards. It would be useful to go from English to French, and not get confused between which kind of brown I'm meant to answer with, whether it would be solved by me merging the cards or by being more specific in the meaning.

Feel free to add synonym detection to our uservoice feature request tracker.
BTW, the way I deal with synonyms is to write:
-marron
-brown (not brun)
