Como hago para que un cuestionario funcione

Necesito que un cuestionario de 11 paginas me funcione algunas preguntas es de falso y verdadero otras de escoger de 1 a 5 necesito que al finalizar me de el resultado de cuantas preguntas respondieron bien.

1)  11 Screens?  The recommendation is to build no more than 10.  App Inventor 2 apps become unstable when using more than 10 screens.
