Search the forum for "login" (or google for "AppInventor Login") - there are many, many methods used

you mean i program the blocks as they are shown in the screen shot?

Create a bound script project on your google sheet and add the script there. Then make sure you publish it to run "as you" with "Anyone, even anonymous" able to access it.
Use the web app url provided in your app.
If you make any changes to the script, republish with new version.
