Where to write for US federal & NY state tax forms?

Could someone tell me what addresses I should write to to order tax
forms and instruction booklets for US federal and NY state income
taxes? This information is proving hard for me to find: the US consulate
here hasn't received the federal forms yet, and knows nothing about
state taxes; and my friends in NYS seem to be all using paid preparers
or waiting until April 14 to get their instruction booklets from the
post office.

Whether you post or not, please email me a copy of any answers: my
news server has limited disk capacity, and whenever it fills up
any extra messages are simply lost and I may never see them.
I looked for a FAQ for this group but didn't see one.
I also looked at the info in http://www.scubed.com:8001/tax/tax.html,
but it doesn't cover NYS. In a pinch, I might get federal forms in
PostScript, but I don't have the right paper size here.
Please remember that I can neither dial a US 1-800 number nor walk
into a US post office from here. I need mail-order addresses for the
forms.
