Canadian income from Investments in Canada

Do permananents residents in the USA need to report income from
Canadian sources mutal funds in Canada?

A Canadian tax form NR4 does this have to be included in a US tax
return?

Yes, with a few exceptions for some items covered in the US - Canada tax
treaaty.

You will also need to work out the exchange rate conversion.

Does the applicable exchange rate depend on the transaction dates, or is
it the rate on a particular date? What if different sources give
slightly different rates? Is there an authoritative source whose rates
must be used?
