Medical Forms 160 and 26 - Name Of Doctor

Hello everyone,

For anyone that has completed Forms 160 and 26, I need some help with
this question:

"6. Name of your doctor"

Is this asking for the name of the Doctor I am getting the medical
examination from, or is it asking for my own doctor that I see?

If it is asking for my own doctor, what if I don't really have a doctor
that I see regularly? I haven't been to a hospital in years, so would I
put "No Doctor"?

Name of your doctor ?
It is not mandatory you can leave it blank.
I also did not mention

You can leave it blank .... It is meant for your family doctor, if you
dont have any then leave it blank.

But it MUST not be the name of the panel doctor through whom you are
going to do your medicals .... otherwise HOC(DIAC) would consider it as
an conflict of interest !
