MV3623 VIN certification form

I have an MV3623 form here from the WI DOT to certify the engine/frame numbers on a '77 RD400. Stopped by my police station (District 5, Milwaukee) and they had never seen it before and didn't feel comfortable signing that the VINs matched.

Has anyone had any luck getting one of these completed by an officer in Milwaukee? They suggested going to the state patrol in Waukesha which is a trip I'd like to avoid.

FWIW, I took a bike through salvage inspection at the Waukesha state patrol office a couple years ago, and it couldn't have been easier. Dude behind the counter filled out all the paperwork before even looking at the bike, very laid back ... not at all the formal, intimidating atmosphere that one might expect from the "STATE PATROL OFFICE".

Probably depends on the officer you get when you stop in, YMMV, and all that, but just thought I'd mention it.

I've done it several ways over the years. 
Once, a long time ago, an officer came out to a friend's garage to verify the VIN for a title transfer from a non-WI title. 
Been to local police departments, sometimes they aren't familiar with all the laws, rules & regulations. 
Lately, I've done the Waukesha inspection for a few bikes. The staff there know the rules.    If  I can make the appointment times, that's my 1st option, now.

In my experience, the county Sheriff is the person to see. They have never come out to me. I have always made an appointment a day ahead of time. I prepare with photos of VIN # and know exactly how to describe their location. I have the paperwork partially filled with my contact info. If a fairly odd and rare bike, i may even bring a photo of a factory model just to highlight my intentions and spark interest. Often times we spend more time talking old bikes than looking a VIN and paperwork. The sheriffs have always wished me luck and alls gone well with Madison DMV for 3 newly titled bikes. mad.d
