New version of the book

Thanks to all of you who have commented or contributed in some way! :-D
We uploaded a new version of the book, available from
http://mikadomethod.wordpress.com/2010/08/04/the-mikado-method-book/
Main changes are:
* "Guidance" chapter has been given more Mikado relation, and we moved
it a bit to the back of the book.
* Jake and Melinda are now our narrators (at least in the beginning of
the chapters) to try to tie it together.
* If you haven't downloaded in a while, you might find that the
program listings are much nicer now.
* Linking within the document looks better.
Please continue to give us comments. If your comments hasn't made it
into the book yet, don't worry. In some cases we are trying to figure
out how to solve things. In other we just haven't gotten around to
them yet. Still, we want to put out a new version about every two
weeks (as it feels now ;-)).
