Medical release form

Hi again.

In the last email sent, I forgot to attach the medical release form for those under 18. If you are under 18 coming to the retreat. You need to print and fill out this waiver (attached) along with a parent's signature of consent for you to participate in the retreat activities. It will then need to be given to the officers at retreat registration. If you have trouble retrieving this, you can find it on midwestcy.org toward the bottom of the page under "Retreat Registration," I will also try to put it up on facebook somehow.  Thanks for the cooperation and God bless,
